If you received an email notice from the Settlement Administrator containing a Notice ID and PIN and your contact information has changed, you may login below to update your name and address information. If you would like to submit a Claim, please click the link to “File a Claim” above.
If you are unable to locate your Notice ID and PIN, or if your name has changed since you were a Northwell patient, please contact the Settlement Administrator by emailing info@NWPixelSettlement.com. Please request your Notice ID and PIN by providing your full name and mailing address.
